Английский. Put the verb forms into the passive voice

They handed me a form (1) where they asked me(2) how i could settle my bill (3). -I was handed a form where I was asked how my bill could be settled.The man to whom they gave my money (4) did not seem to know what it was : nobody had probably taught him(5) at school what we used money for(6).-The man to whom my money was given didn't seem to know what it was :he hadn't been taught at school what we used money for.
