General Visa Requirements

Original passport valid for at least six months
Correct visa fee
One Recent passport-size photograph (five photographs in case of Pakistani nationals)
'Aids-Free' Medical certificate (for more than one year visas) (for Ukrainian nationals only)
Supporting documents, where necessary
Duly completed application form (Pakistani and Bangladeshi nationals need to apply on special application forms). Applicants holding nationality other than Ukrainian should fill out an additional form (Telefax for visa clearance) along with visa application and also attach a copy of the first page of their passport.
Visa Application from the spouse of an Indian national should be accompanied with a copy of the Indian passport (first and the last laminated pages) of the spouse.
All Ukrainian nationals applying for Visa are requested to submit a copy of International Passport, as well as National Passport (first page and registration page) along with the Application Form. Other foreigners, residing in Ukraine, are requested to submit a copy of their International passport along with residential permit/registration issued by Ukrainian government.
